10. If the tax status is type 2 or 4, also provide the market value per share of the new stock on the date of merger (open, average, or closing price.) Use the market value per share that the brokerage firm used to compute the taxable proceeds on your Form 1099. $
11. If the tax status is type 5, also provide the amount of liabilities released (total amount for the tax lot and not per share) at the date of conversion (find this on your Sch K-1.) $
12. If the tax status is type 5, also provide the net amount of income less expenses less distributions reported on your Sch K-1 from your original date of purchase until the date of conversion (total amount for the tax lot and not per share). $
